More About Your Role
  • Work as part of a team delivering range of sustainable Healthy Communities projects/programmes that deliver measurable impact for the community for a local contract area. With a focus on tackling health inequalities.
  • In line with relevant training / qualifications held, deliver specific Healthy community projects/ programmes, including:
    Exercise Referral, Cardiac Rehabilitation, Stroke Rehab and Healthy Communities Classes
  • Complete any monitoring and evaluation requested by line manager in line with current data protection protocols
  • Screen, process and contact referrals to programmes delivered, including conducting telephone consultations with clients
  • Support the team to increase referrals to programmes by attending outreach events and liaising with healthcare professionals and other referring organisations

More About You
  • Qualified instructor in relevant in relevant discipline to deliver sessions independently. Must hold Level 3 GP Referral Qualification and/or be willing to work towards achievement of Level 4 Qualifications including Cardiac Rehab, Neurological Conditions, Cancer Rehab and PSI.
  • Experienced in planning, delivering and monitoring sessions.
  • Passion for improving the health of the local community.
  • Ability to use MS tools.
