Early Support, Ageing Well and Safeguarding, and Disabilities and Mental Health, based on what matters most to the people we support and the professionals who work with them, making sure people receive the right support, at the right time, by the right person.

As a result, we are investing in our social work workforce and have a number of social work posts across the new service areas.
About the Role – Disabilities and Mental Health Keeping in Touch (Review) TeamThis is an exciting opportunity to join a new team in our Disabilities and Mental Health Service.
You’ll be part of a forward-thinking, compassionate service that values your voice, nurtures your development, and champions your wellbeing, where no one day is the same.
In this rewarding role, you’ll be carrying out conversations to keep in touch (review) with people who have care and support in place. This will be an opportunity to:
- Use your creativity to ensure people with disabilities and mental health needs are being supported to live gloriously ordinary lives, and are connected to their communities
- Ensure people’s voices are heard and represented in care and support plans
- Ensure young adults have the support they need to navigate early adulthood with confidence to look to the future
- People at risk of being, or who have been, involved with the criminal justice system, working alongside them to continue to build a good life based on care and support plans that promote safety and independence
We use The Three Conversations, a strengths-based, person-centred approach, that helps us think about: “What does a good life look like to you?” From there, we work together to ensure the support we offer is tailored, empowering, and safe, promoting choice, dignity, wellbeing, and independence.

You will:
- Be dedicated to and passionate about working with people with disabilities and mental health needs and their carers / families
- Have a sound working knowledge of relevant legislation
- Have strong analytical, assessment and negotiation skills
- Be passionate about strengths-based working
- Be able to demonstrate you can work with colleagues and alongside service providers, partners and commissioners
- Have strong interpersonal, communication and IT skills
- Be registered with Social Work England
- Be an Approved Mental Health Professional/Best Interests Assessor or eager to undertake training
- Have a vision for your own development
- Have completed the ASYE or an equivalent post-qualifying pathway
