Occupational Therapist - Community Reablement

Community OT Team - Pathway 1 Reablement (Wolverhampton) A service in Wolverhampton is seeking an experienced Senior Occupational Therapist to join their Community OT Team supporting Pathway 1 Reablement. This is a 12-week role working 3 days per week, focused on supporting individuals in the community to regain independence following hospital discharge.

• Provide occupational therapy assessments within the Pathway 1 Reablement service
• Support individuals to regain independence at home following hospital discharge
• Carry out functional assessments and develop goal‑focused reablement plans
• Recommend appropriate equipment, adaptations, and interventions to promote independent living
• Work collaboratively with multidisciplinary teams including health and social care professionals
• Manage and prioritise a community‑based caseload
• Support discharge pathways and help prevent hospital readmissions
• Provide professional advice and guidance to colleagues within the service

• Diploma/Degree in Occupational Therapy
• HCPC Registration
• Previous experience working in community or reablement services
• Experience completing functional assessments and supporting independence at home
• Ability to work independently while contributing to a multidisciplinary team
• Strong communication, organisational, and clinical decision‑making skills
• Ability to travel within the community as required
