• Monitor the wellbeing of each resident, identify changes impacting care plans, and ensure that new needs are met, updating written records accordingly.
• Participate in the person‑centred care planning process, implementing, monitoring, and undertaking monthly evaluations.
• Support residents with medication requirements.
• Conduct staff supervisions and support staff development as needed.
• Act as the key point of contact with residents, their relatives and visitors, and with health and social care professionals involved in their wellbeing, ensuring reasonable requests are acted upon and concerns or complaints are escalated.

• NVQ Level 3 or equivalent in care for care homes.
• Minimum of two years experience working in a similar care environment.
• Excellent verbal and written communication skills.
• Positive attitude toward supporting and nurturing residents and staff.
• Responsive and flexible, able to cover a range of responsibilities.

• DBS Disclosure check, cost covered by Avery Healthcare.
• Proof of eligibility to work in the UK.
