Help patients get home safely and sooner by keeping our wards moving. We are looking for a motivated and compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team at Paulton Memorial Hospital. The role supports safe, timely discharges into community services across BSW Community Hospitals, working closely with nurses, therapists, social workers and other members of the multidisciplinary team.

Location

Paulton Memorial Hospital is located in the heart of Paulton, Somerset, close to the Mendip Hills. The area offers rural charm with good transport links to Bath, Bristol and surrounding towns and plenty of local amenities.

Responsibilities
  • Support safe and timely patient discharges by completing all required administration early and accurately.
  • Visit patients on wards to update them and their families on discharge plans.
  • Help patients and families understand discharge processes, expected time frames and the role of the Flow team.
  • Attend MDT meetings and record actions, updates and discharge readiness on clinical systems.
  • Attend professional and family meetings and capture clear, accurate minutes.
  • Monitor predicted discharge dates for all patients and highlight changes or concerns to the team.
  • Ensure patients are registered on the correct clinical systems and that electronic records are kept up to date.
  • Use multiple digital systems confidently to support smooth, timely discharges and avoid delays.
  • Identify patients who may need complex discharge planning and raise this with the Senior Coordinator.
  • Ensure all referrals are completed before discharge and escalating where actions are outstanding.
  • Maintain excellent communication with the MDT, system partners and commissioners.
  • Provide clear information leaflets to patients, families and carers when needed.
  • Answer calls and relay messages professionally, respectfully and accurately.
  • Collect data and support required reporting for the service.
  • Promote good communication within the MDT and ensure predicted discharges are shared daily.
  • Act as a point of information on available services and referral pathways.
  • Keep mandatory training up to date and contribute to a positive, proactive team culture.
Person Specification
  • NVQ Level 3 in Care, Business Administration or equivalent experience.
  • Relevant IT skills – Microsoft Office, Excel, Word, Email, MS Teams.
  • Experience working in a health or social care setting.
  • Experience working within a multidisciplinary team.
  • Strong organisational skills with the ability to prioritise workload.
  • Minute‑taking and note taking experience.
  • Professional and empathetic communication with patients, families and colleagues.
  • Ability to work well in a fast‑paced environment and remain calm under pressure.
  • Positive, caring attitude.
  • Effective communication skills in person and by phone.
  • Experience in discharge planning.
  • Proactive, motivated and open to learning and improvement.
  • Ability to plan and deliver work over extended periods within agreed time frames.
  • Willingness to travel between sites.
Desirable
  • Experience using health and social care systems such as Systm One.
  • Knowledge of discharge planning processes.
  • Awareness of infection control guidance.
  • Experience in a hospital, community or adult social care setting.
