Oxford Health NHS Foundation Trust is seeking a part-time Social Work Assistant (22.5 hours per week) to assess needs under the Care Act and review care packages with carers. The role sits within the Adult CMHT, with training and development opportunities from Oxford Health and Buckinghamshire Council.
You will work with a multi-disciplinary team, arranging services as needed, and supporting older adults and families to live independently. A driving licence and vehicle access are required.
