Cardiac Rehabilitation Sister/Charge Nurse
We are looking for a highly motivated, enthusiastic nurse with proven specialist knowledge and experience in cardiology to work in our established multi-disciplined Cardiac Rehabilitation Team at The Royal Wolverhampton NHS Trust. The post holder will work between the two hospital sites at New Cross & Cannock Chase Hospital, and in our community venues, as required. This is an exciting opportunity to improve the clinical outcomes and quality of life for our service users who have recently received treatment for their coronary artery disease (CAD).
Our role involves being able to act as a clinical expert for our primary and secondary health care teams and effective liaison with other community health care professionals. We are seeking those with a high level of knowledge of cardiovascular disease (CVD) in order to provide clinical expertise in providing all aspects of coronary heart disease management, including audit and development of the service.
duties of the job
- The post holder will be an experienced nurse contributing to the development, delivery and evaluation of a multidisciplinary, menu-based cardiac rehabilitation service.
- To work autonomously taking responsibility for care, quality and clinical activity for a designated caseload of patients.
- To process referrals appropriately, ensuring that patients are seen within designated waiting times and that tasks are delegated to administration staff effectively and efficiently and communicated to wider multidisciplinary team.
- To triage and prioritise patients, demonstrating the use of a variety of techniques to elicit the history of an illness and determine an appropriate care plan.
- To take responsibility for direct patient care: assessing, planning, providing and evaluating nursing care needs including monitoring, diagnostics and investigations.

- Lead a team and co‑ordinate the clinical area in the absence of the Senior Sister / Charge Nurse and reinforce the values and standards of compassion, dignity and respect in care, leading the delivery of clinically excellent care.
- Ensuring the planning and delivery of care is aligned to professional values of care, compassion and respect, act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence based practice.
