Digital Care Navigator
Hours – 37 hours per week
Contract – Full time, 1 year Fixed term
Salary – Grade 6 (£36,363 - £40,777)
The Role
This is a specialist role requiring confidence, credibility, and competence in both adult social care and digital transformation. You will work directly with people, carers, and practitioners to ensure that technology supports independence, dignity, and choice—never replacing the human touch, but enhancing it.
Key Responsibilities- Advise, guide, and influence social workers, care coordinators, and provider organisations on the effective application of digital solutions in care planning.
- Carry out person‑centred assessments to identify suitable technology‑enabled care (TEC) options tailored to individual needs.
- Provide training and support to people with a range of abilities—ensuring confidence, understanding, and sustained use of digital tools.
- Act as a link between frontline services and digital innovation, feeding back insight, challenges, and impact to drive system‑wide learning.
- Significant experience working in a professional adult social care, health, or related setting.
- A strong track record of using digital care or assistive technology to support people with complex needs.
- Experience delivering training or coaching to people who use services, their carers, or frontline practitioners.
- A confident, organised, and proactive approach to strengths‑based and strategic work across teams and agencies.
- A genuine commitment to co‑production, inclusive communication, and listening to lived experience.
