Service Care Solutions are recruiting on behalf of Medway Council for an experienced Social Worker to join the Children and Young People Disability Team. You will deliver high-quality assessments, safeguarding interventions and transition planning while working with disabled children, young people and their families.
You will use Signs of Safety in assessments, prepare reports for meetings, and maintain accurate records.
