LPN — Resident Care, Care Plans & Growth

Nexus Health is seeking a Licensed Practical Nurse (LPN) to provide high-quality resident care in a long-term care setting. You will assess patient needs, implement care plans, and collaborate with the care team to achieve optimal outcomes.

Responsibilities include administering medications, monitoring progress, and educating patients and families on health conditions and post-care instructions. Illinois LPN license and CPR certification are required, with at least one year of LTC experience.
