- Woodbridge, VA~Full Time & Day shift~Monday
- Friday: 8:00am-4:30pm, rotating weekends and holidays
40 hours/week

Overview

Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.

Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings:
Hospitals Only.

Certification in Case Management (CCM, CMAC, or ACM-SW) is required within one year of eligibility.
1 year of acute care and/or healthcare experience requirement is defined below:

Educational internship experience in an acute care facility can be considered.

Or Healthcare experience refers to the care delivered in one of the following settings: long-term care, home health, hospice, SNF.BLS is required within 90 days of hire.

EducationMLD – Master’s Level Degree – Social Work Certification/Licensure Basic Life Support (BLS) - Certification
- American Heart Association (AHA)

Or Healthcare experience refers to the care delivered in one of the following settings: long-term care, home health, hospice, SNF.Previous acute care experience (preferred). The base pay range for full-time employment is $89,544.00 - $ annually. Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.

Note:

If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
