Home Care RN to Focus Clients not Paperwork

Perform routine scheduled visits with clients to monitor care and oversee CNAsand caregivers in the home. Maintain detailed and oversee care plans. Assist Clinical and Marketing Teams when your expertise is needed.

• Conduct Weekly and Bi-Weekly visits to Clients
• In person home visits as needed within 2hr time period
• New clients assessments in a timely manor
• Maintain accurate and current care plans
• Coordinate Client needs with their PCP as needed
• Provide counsel and guidance to family members on managing conditions

• Active RN License in Virginia or a compact state
• Driver's License and vehicle
• Current CPR
• One year in home care experience required
