Legacy is recruiting an Outpatient Nurse Practitioner or Physician Assistant for our Woodburn Health Center, located at 1475 MOUNT HOOD AVE., WOODBURN, OR (Qualifies for Legacy’s Special Location Allowance stipend). Other providers in this clinic include physicians and APPs, all of whom have been a part of Legacy for several years. Woodburn is a vibrant community located one hour south of the Portland Metro area in the beautiful Willamette Valley.
Our primary care providers are supported by individually assigned MAs as well as care teams that can include panel coordinators, social workers, RN case managers, and behavioral health clinicians and pharmacy.
Our clinics support the local communities by providing services to diverse patient populations. With more than 25 clinics across the Portland metro area and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy.
Responsibilities- Responsible for the patient throughout the course of treatment, including reviewing clinical history, providing the physical examination, making the diagnosis, and treating both chronic and episodic disorders.
- Work in collaboration with the team and seek the advice of a team physician whenever a case falls outside the scope of expertise.
- Maintain own panel of patients and share call coverage for the clinic.
- Graduate of Nurse Practitioner or Physician Assistant program.
- Minimum of 2-years’ experience.
- Current Oregon NP or PA license w/Prescriptive Privileges.
- Current BLS certification from the American Heart Association
- A nonprofit that serves the community.
- Community leader in providing healthcare and healthcare related services to the underserved.
