Responsibilities

This position will work primarily with adult patients with co-occurring disorders.

Hours are Monday - Friday, 8a-4:30p.

Responsibilities and Accountabilities:

  • Accountable to have knowledge and understanding of Social Work practices and principles
  • Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psycho educational and/or therapeutic groups
  • Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
  • Responsible for communicating with patients and families to keep them involved in treatment plan decisions
  • Coordinates care with outpatient providers upon admission and discharge
  • Responsible for facilitating weekly treatment team meetings and creating master treatment plans/updating treatment plans as agreed upon by the treatment team
  • Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
  • Communicate clearly and effectively with clinical team members and Directors
  • Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department
Qualifications
  • Master’s Degree in Social Work preferred, with licensure in the State of MN required
  • Two years' experience in a mental health care setting preferred.
  • Experience working with co-occurring disorders strongly preferred.
  • Experience using electronic health records preferred.
  • Knowledge of Microsoft Office products including Word, Outlook, and Teams.
  • Ability to communicate in written and verbal form effectively and efficiently.
  • Ability to coordinate care with relevant staff.
  • Ability to shift between dissimilar tasks smoothly and rapidly.
  • Ability to make effective decisions while in stressful situations and circumstances

Compensation for this role ranges from $29- $36 per hour depending on licensure and experience.
