Hospice Admissions Nurse – Field

Job Title: RN Admissions – Join Minnesota’s Most Person-Centered Hospice

Compensation: $200 - $250 Per Admission

Benefits: Full Benefits Package (Full-Time only)

Location: Greater Twin Cities Metro – 21-county area

Schedule: Full-Time or Part-Time | Includes 1 on-call shift per week (backup) and every 4th–7th weekend

This is a mobile, high-touch position. You’ll meet patients in homes, hospitals, or senior living settings to assess eligibility and educate families about hospice services.

• Completing patient assessments and initiating care
• Offering families a steady hand during emotional decisions
• Communicating with physicians and team members
• Accurate charting and efficient case handoff

• Licensed RN in MN
• Experience in hospice, oncology, or end‑of‑life care
• Ability to connect and comfort
• Skilled in time management and prioritization
• Dependable transportation and driver’s license
