Job Overview: To appoint a licensed Psychiatrist or Registered Nurse Practitioner (RNP) to provide psychiatric care, consultation, and medication management services for the residents of Shady Lane Nursing Home. The provider will ensure compliance with all federal, state, and facility regulations, deliver routine and consultative psychiatric care, and support resident and staff well‑being through evaluations, counseling, and medication oversight.

• Provide psychiatric assessments, medication reviews, and counseling for residents.
• Evaluate all new admissions with psychiatric diagnoses or medications.
• Conduct annual evaluations for residents with psychiatric, Alzheimer’s, or dementia‑related conditions.
• Offer grief counseling services to residents and staff as needed.
• Collaborate with facility leadership on vacation coverage and service continuity.
• Ensure accurate documentation and compliance through the Sigma Care System.
• Maintain all licenses, certifications, and malpractice insurance during the contract period.

• Resident Psychiatric Care:
  Provide routine and consultative psychiatric services to all eligible residents.
• Conduct psychiatric assessments for new admissions with psychiatric diagnoses or medications.
• Perform yearly evaluations for residents with psychiatric, Alzheimer’s, or dementia diagnoses.
• Deliver grief counseling for residents and staff as required.
• Perform regular medication reviews and ensure documentation of titration efforts at least twice per year to maintain the lowest effective dose.
• Document reasons when titration is not performed.
• Ensure safe, compliant, and effective psychopharmacological management aligned with state and federal guidelines.
• Record all orders, evaluations, and progress notes in the Sigma Care System.
• Maintain compliance with federal, state, and local mental health regulations.
• Notify the Facility Administrator and Director of Nursing of any changes in coverage or service delivery.
• Maintain active NJ medical or advanced practice license and malpractice insurance.
• Directly invoice residents, Medicare, Medicaid, or managed care organizations in accordance with federal and state laws.
• Collaborate with facility administration for coverage coordination during absences or vacations.
• Licensed Psychiatrist or Registered Nurse Practitioner (RNP) in the State of New Jersey - Required
• Active Malpractice Insurance - Required
• Experience with long‑term care residents (psychiatric, Alzheimer’s, or dementia populations) - Required
• Familiarity with Sigma Care System or similar EHR platforms - Preferred
• Strong understanding of federal, state, and local psychiatric care regulations - Required
• Excellent communication, documentation, and interdisciplinary collaboration skills
