At Home Liaison Homecare - VHS-FT

We are looking for a dedicated professional to serve as the primary point of contact for patients and families, ensuring a smooth transition between home care services and our organization. The role involves multi‑site coordination, cross‑selling of services, and delivering durable medical equipment.
Responsibilities- Represent the organization and provide initial introductions to all service lines and products.
- Identify opportunities for cross‑selling and deliver appropriate collateral to educate and promote the organization.
- Gather patient information from charts and computer systems and forward it to Central Intake.
- Assist in the face‑to‑face process by obtaining and sending required documents.
- Build ongoing relationships with case managers, social workers, physicians, and other staff through excellent customer service.
- Maintain communication with organizational operations and utilize the tracking system to follow up on pending cases.
- Act as a liaison between patients/families and the organization, welcoming and explaining product lines during initial visits.
- Process DME (where applicable) and assist with collecting & sending documentation.
- Assist in obtaining and sending signed face‑to‑face forms.
- Perform other duties and projects as assigned to support departmental functions.
- Adhere to HMH’s organizational competencies and standards of behavior.
- Lift a minimum of 10 lbs., push/pull a minimum of 10 lbs., and stand for a minimum of 4 hours per day.
- High school diploma, GED, or equivalent.
- Minimum of one (1) year of customer service experience.
- Proficient in Microsoft Office Suite.
- Strong customer service skills.
- Homecare experience.
- Sales experience.
- 2 to 4 years of healthcare experience.

Starting hourly rate: $23.97. Additional compensation may be based on experience, education, skill set, geographic location, and other factors as described in the job duties section.
