Community Health RN Coordinator

Under році direction of the Community Health and Outreach Director, the Community Health RN Coordinator (CH/OR RN Coordinator) provides leadership to regional teams who support community health planning activities, agency and community relations, resource development for community health improvement projects, and related work as required. This position is responsible for working with the community and managingèche regional CH/OR teams to support community health planning and/or activities to improve the health of Native Americans in the NVIH service area.

ESSENTIAL

JOB FUNCTIONS
  • Provides care management services to patients with complex health care needs and creates a patient‑centered self‑management plan that ensures access to.forward medical, dental, & behavioral health services and lifestyle interventions to improve health outcomes.
  • Provides team and regional leadership and support to CH/OR team, including but not limited to staff evaluations, coaching/counseling, motivates and inspires staff to work as a cohesive and productive team.
  • Ensures CH/OR programs within the region are functioning to meet the expectations of the program and the community.
  • Provides oversight and supervision for day‑to‑day programming, including onboarding of new CH/OR staff, monitoring/auditing caseloads of CH/OR team members to provide guidance and support for complex cases, and supporting case‑conference meetings.
  • Provides effective support to patients and works with NVIH staff and the community to support all phases of public health planning—assessment, planning, implementation, and evaluation—applying interpersonal skills to enhance collaboration of NVIH staff, community members and agencies toward common health outcome improvement goals.
  • Plans and participates, in collaboration with NVIH staff, to obtain input for community health and outreach planning activities, such as health fairs, health events, and health educational programs.
  • Facilitates and participates in program quality improvement projects, as well as Government Performance Results Act (GPRA) activities to improve program workflows and patient health outcomes.
  • Assists with preparation of grant applications or funding proposals and to promote the development of resources to support innovative population‑based health promotion efforts.
  • Assists in reviewing and revising policies and procedures.
  • Other duties as assigned.
REQUIRED QUALIFICATIONS
  • Valid and current California Registered Nursing License.
  • BSN or PHN certificate preferred.
  • One (1) year experience as an RN.
  • Demonstrates clinic knowledge and skills that will be essential in conducting health assessments and developing a patient‑centered health plan in accordance with best practice standards.
  • Ability to learn, adapt, supervise and/or direct others, display concern and initiative, and be resourceful and calm in emergencies.
  • Clinical leadership experience preferred.
  • Profic 이상예hunt case management skills to support patients and collaborate with providers on complex patient cases.
  • Demonstrates ability to think creatively and critically to support various initiatives.
  • Effectively communicates in a clear, concise manner, including but not limited to public speaking.
  • Demonstrates ability to collaborate effectively with multidisciplinary team members and community health organizations to ensure effective coordination of patient care.
  • Demonstrates understanding of age‑related or specialty issues​ and developmental needs for each population served by adapting style, treatment and plan to each population.
  • Ability to use electronic health records and maintain current, accurate medical records, charts and health plans. Familiarity with Microsoft applications, such as Outlook, Excel, Word, & Teams.
  • Works independently under limited supervision, prioritizes work assignments, shows excellent organizational skills andyna works well under pressure of deadlines.
  • Ability to travel and operate GSA vehicle for business purposes.
