Job Description

• Manage 25-30 C&C active cases (mainly low-moderatelevel of complexity) per quarter.
• Conduct assessment,planning and intervention for cases bytaking into account the integration of a range of information, including practice guides and organisational frameworks.

Conduct case conferences with partners.

• Participate in networking and case management meetings.
• Document and evaluate case outcomes.
• Develop and review appropriate intervention strategies.
• Ensure proper documentation of cases.
• Ensure reports on cases aresubmittedin a timely manner.

• Manage case referrals.
• Part of the Intake & Triage Team to conduct intake assessments for referred clients based on the practice standards of the Code of Social Work Practice and Practice Guide.
• Refer clients to appropriate agenciesfor further intervention.

• Network with other organizations and stakeholders.
• Participate in implementing and evaluating of community outreach/ initiatives.

• Be part of the programme team for Group Work conducted at the FSC.

• Degree or Graduate Diploma in Social Workor related field such as Psychology, and other relevant fields in Social Science
• Previousexperiences via internships or placements in related areas of work will be considered
• Understanding of social work values and ethics
• Understanding of work with vulnerable populations i.e. low-income families
