About the Role
Ascend Medical is seeking a warm, experienced therapist with a passion for working with older adults to join our Behavioral Health Team. This independent contractor position is ideal for a Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) who enjoys building relationships and delivering impactful care on-site within senior living and long-term care communities.
Responsibilities
- Provide in-person therapy to older adult patients in assisted living communities.
- Conduct psychosocial assessments and clinical diagnoses using DSM-5-TR.
- Develop and update personalized treatment plans every 90 days or as clinically indicated.
- Deliver evidence-based therapeutic interventions tailored to geriatric populations.
- Address mental health challenges such as anxiety, depression, grief, adjustment disorders, cognitive decline, and caregiver fatigue.
- Collaborate with on-site care teams, including medical providers and facility staff.
- Session documentation, consistent with best practices and insurance reimbursement standards completed in Athena (EMR) within 24 hours of service delivery.
- Maintain accurate, timely clinical documentation and uphold all licensing standards.
Qualifications
- Active licensure in Georgia (LPC, LCSW, or LMFT); additional state licensure is a plus.
- Demonstrated experience in geriatric care, ideally within senior living or skilled nursing environments.
- Experience facilitating group therapy sessions, with the ability to adapt approaches to diverse senior populations.
- Strong interpersonal skills and comfort working independently within on-site settings.
- Active professional liability insurance.
- Passion for whole-person, compassionate care.
Pay range and compensation package
This role will begin at 10 hours per week, split between two senior care communities:
- 5 hours per week at Woodstock
- 5 hours per week at Acworth
Compensation is $81 per completed patient visit. To support onboarding and initial patient scheduling, there will be a 90-day guaranteed minimum of 10 visits per week (equivalent to $810 weekly).
