Responsibilities- A Home Therapy nurse trains home hemo or peritoneal dialysis patients in‑center and sometimes at the patient’s home.
- In‑center hemodialysis, critical care, and home health background is desirable.
- Work schedule is Monday‑through‑Friday, 8 am to 5 pm with potential modified schedules based on patient caseload.
- Competitive on‑call pay when placed in on‑call rotation.
- Home Training Nurses rarely work on a holiday.
- Lead the industry in clinical quality improvement and deliver outstanding patient outcomes as measured by the CMS ESRD Quality Incentive Program.
- Provide patient training and ongoing support for all patients choosing a home dialysis modality.
- Ensure compliance with federal, state, and local laws and regulations related to home dialysis program and patient care.
- Train patients (and/or care partners) in the practice of self‑care Peritoneal Dialysis or Hemodialysis.
- Conduct home visits to assess the patient’s home environment and evaluate performance.
- Communicate assessment results to physicians and team members.
- Document all nursing services in the Electronic Medical Record.
- Participate in infection control monitoring, implementation, and recording.
- Be familiar with emergency equipment and procedures.
- Maintain clinic and patient supply inventory.
- Track and trend program and patient performance for continuous quality improvement.
- Obtain routine and non‑routine laboratory tests and communicate critical values.
- Complete monthly review of patient medication profiles and administer medications as ordered.
- Perform on‑call nursing services nights and weekends on a rotational basis.
- Be flexible with staffing locations and hours to accommodate program needs.
- Maintain regular and reliable attendance.
- Coordinate home evaluation with technical services to ensure electrical, plumbing, and drainage requirements are met.
- Ensure required water sampling is completed per policy and monitor/report water sample results.
- Current RN license in applicable state, maintained in good standing.
- 12 months of clinical experience and 3 months of experience with the specific modality (6 months in Texas). Preferred: experience with patients on maintenance dialysis.
- CPR certification required prior to patient care assignment.
- Ability to distinguish all primary colors.
- Successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency.
- Working knowledge of English and ability to communicate verbally and in writing.
- Basic computer skills; proficiency in USRC clinical applications required within 90 days of hire.
- Meet any practice requirements for the applicable state.
- Meet minimum hiring standards, including background clearances and orientation requirements.
