Physical Therapist at Enhance Rehabilitation

Enhance Rehabilitation is currently seeking qualified Physical Therapists to join our dynamic team. As a Physical Therapist, you will evaluate and treat patients in a skilled nursing facility (SNF) setting, developing individualized plans of care.

• Provide excellent care for our patients
• Deliver exceptional customer service to patients and family members
• Collaborate with facility staff to ensure quality care
• Evaluate new patients, develop care plans, and set measurable functional goals
• Treat patients, address concerns, and educate them on injury, progress, and outcomes
• Supervise Physical Therapist Assistants
• Re‑evaluate patient progress, revise care plans, and reset goals
• Complete timely and accurate documentation (evaluations, re‑evaluations, discharges, daily progress notes)
