- DRIVER'S LICENSE & UTILITY BILL MUST BE INCLUDED WITH SUBMISSION
- NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED

- Experience required:
Pharmacist performing inpatient duties as well as preparing outpatient infusions and chemotherapy. - Certs/Licenses: BLS, PharmD, RPH
-Bachelors degree in Pharmacy
* Current Oklahoma Pharmacy license - Chemo/Hazardous Drug Certification preferred
- IV Certification preferred.
- Will clinician float within scope to meet facility needs? Yes
