Pace Ri is seeking a Home Care Registered Nurse to assess participants in the community, develop and update care plans, and coordinate with the IDT and PCP. The role requires clinical nursing skills, documentation, and effective communication with families and team members.
The RN will ensure safe transitions of care, coordinate home services, and participate in staff meetings. Weekend/holiday coverage and driving to participant homes are involved per schedule.
