Job Summary:

We are seeking a Registered Nurse (RN) to join our Home Health team on a PRN (as needed) basis in Wayne County. The RN will provide compassionate and high-quality care to patients in the comfort of their homes, ensuring their well‑being and promoting independence.

Qualifications:

• Valid RN license in the state of Michigan
• Minimum of 1 year of nursing experience, preferably in home health or a similar setting
• Strong clinical skills and the ability to assess, plan, implement, and evaluate patient care
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Reliable transportation and a valid driver's license
• CPR certification

Responsibilities:

• Conduct home health assessments and develop personalized care plans for patients
• Administer medications, treatments, and procedures as prescribed by the physician
• Monitor patient progress and report any changes to the healthcare team
• Provide patient and family education on disease management and self‑care techniques
• Coordinate with other healthcare professionals to ensure holistic care for patients
• Maintain accurate and timely documentation of patient care
