Obesity Medicine Physician – Dedicated Collaborator

Join Form Health, a virtual obesity medicine clinic delivering evidence‑based treatment via telemedicine. We are seeking an ABOM‑certified physician to provide direct clinical services and structured supervision for up to three advanced practice providers (APPs). This role focuses on collaborating with APPs to co‑manage a shared panel and enhance team consistency, safety, and effectiveness.

• Provide direct patient care focused on evidence‑based obesity treatment and chronic weight management, collaborating with APP colleagues to co‑manage a shared panel.
• Review charts and conduct return visits for APP patients every third visit to ensure quality and continuity of care.
• Provide treatment planning and medical management support for complex cases or escalated concerns.
• Prescribe non‑controlled obesity medications for patients under direct care and in coordination with APPs.
• Conduct weekly 30‑minute collaborative meetings and 30‑minute asynchronous communication time with each assigned APP.
• Ensure clinical documentation, protocols, and treatment pathways are consistently followed within the team.
• Assist in the professional development of APPs by fostering clinical growth, offering feedback, and supporting continuing education.
• Participate in team‑based care initiatives and contribute to a culture of collaboration and high‑quality care.
• Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.

• MD or DO degree, board‑certified and diplomate of the American Board of Obesity Medicine (ABOM).
• Minimum 5 years as an attending physician in a primary specialty.
• Licensed to practice and prescribe legally in MA and has or will have credentialed with insurance payers; preference for multiple state licenses.
• Excellent interpersonal skills for patient and team interactions.
• Strong interest in mentoring, teaching, and collaborating with APPs.
• Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting.
• Adherence to evidence‑based obesity treatments, behavioral interventions, nutrition, and pharmacotherapy.
• Committed to delivering patient‑centered, culturally competent care.
• Comfortable working in a multidisciplinary team including dietitians, behavioral health providers, and nursing staff.
