Job SummaryA Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
Qualifications- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or equivalent.
- Currently licensed in the state(s) in which practicing.
- Two (2) years’ experience (preferred).
- Acceptance of agency philosophy and goals.
- Ability to exercise initiative and independent judgment.
- Strong interpersonal and communication skills.
- Understand and adhere to established policies and procedures.
- Provide physician prescribed physical therapy and work towards improving or minimizing residual physical disabilities of the patient.
- Return the individual to optimum and productive level within the patient’s capabilities.
- Participate with all home care personnel in patient care planning.
- Direct and supervise personnel as required.
- Responsible for initial assessment, plan of care, maintenance program development, modifications and reassessments every 30 days.
- Perform all skilled procedures as ordered by physician.
- Consult with physicians regarding changes in treatment and write progress reports.
- Instruct patients and family/significant others in home programs and activities of daily living.
- Participate in in‑service programs and present as assigned.
- Participate in quality assessment and performance improvement activities as assigned.
- Attend all patient care conferences as scheduled.
- Prepare clinical and progress notes and assist physician with evaluating level of function.
- Help develop the plan of care and revise as necessary.
- Consult with family and agency personnel.
- Complete and submit OASIS assessments, reassessments, transfers, resumptions, discharges and significant change in condition per agency timelines.
- Appropriately utilize ICD‑10 codes.
Work indoors in agency office and patient homes, traveling to and from patient homes.
Job RelationshipSupervised by:
Director of Clinical Services/Clinical Manager
Workers supervised:
Physical Therapy Assistant and Home Health Aide, as assigned.
High risk.
Lifting RequirementsAbility to perform the following tasks if necessary: participate in physical activity, work extended periods standing and engaging in physical activity, heavy lifting, extensive bending, lifting, and standing regularly.
