Description:
Effectively screens, evaluates, and rehabilitates patients with physical functioning disorders and/or wound care.
Develops appropriate plan of care following all regulatory and clinical practice standards.
Utilizes standardized assessments and evidence-based practices to support clinical interventions.
Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and use of supportive and assistive devices.
Assesses patients' progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
Evaluates treatment outcomes for effectiveness.
Modifies services in relation to patient evaluation and makes referrals based on outcomes.
Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements.
Documents all regulatory and clinically required information in the electronic documentation system.
Maintains patient records according to company policy.
Accurately records treatment minutes for services provided.
Participates in required meetings, including patient care conferences, utilization review meetings, and rehabilitation meetings.
Participates in required trainings, in-services, and conferences.
Able to obtain necessary durable medical equipment or supplies per regulatory and clinical practice requirements.
Able to supervise and support physical therapist assistants, rehabilitation aides, and/or students.
Adheres to the APTA and state-required practice acts, codes of conduct, and ethics during professional practice.
