Position: RN or LPN Acute 24 Hour Care, Detox, 24 Hrs, Day Shift

** LPN Requirements:
**** I. Major Responsibilities:
** 1. Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, take and monitor vitals of clients on the unit, medication administration and documentation.
2. Renders and supervises care of clients on the unit.
3. Monitors medication and consultation with prescribers.
4. Provide appropriate health care and coordinates referrals as prescribed by providers.
5. Participates in the evaluation of client’s condition and progress with team members.
** Specialty Responsibilities:
**** Charge Nurse:
** as assigned by program leadership. When assigned charge duties you are expected to:
1. Maintain overall shift operation in a safe, efficient and effect manner in the absence of management.
2. Direct and supervise staff including all RNs, LPNs, Contingent workers and Direct Care Staff/ RCs to ensure the delivery of safe and effective patient care.
3. Report any urgent issues on the unit to the management team in a timely manner and utilize the administrator on call for any emergency, medical, and disciplinary situations that require further intervention.

All responsibilities are essential job functions.

Qualifications:

Education:

* Required:

** 1. Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
2. Satisfactory CORI background check.
3. Driving is not a requirement

* Required:

** 1. Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
2. Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.
** Preferred:
** 1. One (1) year of nursing experience
2. Knowledge of medical, substance abuse, and mental health nursing.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
** III.  Physical Demands and

Environmental Conditions:

** 1. Must be able to move about the space periodically during the shift.
2. Must be able to stoop, crouch, kneel or crawl for various reasons
3. Must be able to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medicals codes or alarms).The characteristics above are representative of those encountered while performing the essential functions of the position. ** Registered Nurse Requirements:
**** I. Major Responsibilities:
** 1. Utilizes the Nursing Process for the delivery and management of client’s care from Admission to Discharge, appropriate to the developmental age of the client population.  2.
Performs client assessment /reassessment according to standard procedure.  3.
Develops and coordinates with the interdisciplinary team on the individual plan of care including discharge planning.  4.
Directs, supervises, coordinates and evaluates nursing care within the clinical setting.  5.
Delegates appropriately to others in accordance with the job description and competency level.  6.
Documents client care according to standard procedure.  7.
Develops, implements/coordinates and documents education provided to client/family/significant other.  8.
Ensures quality and continuity of care through collaboration with other healthcare providers from the interdisciplinary team.  9.
Administers medications, performs treatments, therapies, and operates equipment according to policy and procedure.  Assists licensed healthcare provider with treatments, procedures and special tests as required and directed.  10.  Assumes responsibility for professional growth. …
