Job Summary

Provide nursing care for a vulnerable adult population. Perform nursing assessment, treatment, education and health care service coordination to individuals in need. The Acute Care LPN collaborates with all components of the team and other health care agencies to ensure coordinated, comprehensive services.

Schedule and Compensation

Exemption Status: Non-Exempt

Hiring Range: $32.91 - $40.70 (final offer may vary within this range based on a candidate's experience, skills, qualifications, and internal equity considerations)

Schedule Details: Weekends - Every Other Weekend

Major Responsibilities
  • Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration and documentation.
  • Render and supervise care of clients on the unit.
  • Monitor medication and consultation with prescribers.
  • Provide appropriate health care and coordinate referrals as prescribed by providers.
  • Participate in the evaluation of client’s condition and progress with team members.
  • Promote education and adherence to prescribed treatment protocols.
  • Facilitate wellness and medically related psycho educational groups to individuals.
Specialty Responsibilities – Charge Nurse
  • Maintain overall shift operation in a safe, efficient and effective manner in the absence of management.
  • Direct and supervise staff including all RNs, LPNs, contingent workers and Direct Care Staff/RCs to ensure the delivery of safe and effective patient care.
  • Report any urgent issues on the unit to the management team in a timely manner and utilize the administrator on call for any emergency, medical, and disciplinary situations that require further intervention.

  • Comply with established departmental policies, procedures, and objectives.
  • Attend variety of meetings, conferences, seminars as required or directed.
  • Demonstrate use of Quality Improvement in daily operations.
  • Comply with all health and safety regulations and requirements.
  • Respect diverse views and approaches, demonstrate Standards of Respect, and contribute to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.
  • Maintain regular, reliable, and predictable attendance.
  • Perform other similar and related duties as required or directed.

Position Qualifications

License/Certification/Education – Required

  • Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.
  • Satisfactory CORI background check.
  • Driving is not a requirement.

Experience/Skills – Required

  • Demonstrate proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.
  • Demonstrate ability to communicate effectively and professionally with clients, staff, and providers.

Experience/Skills – Preferred

  • One (1) year of nursing experience.
  • Knowledge of medical, substance abuse, and mental health nursing.
