• Ability to assess the healthcare needs of the patient/resident, make a nursing diagnosis, transition this through collaboration with the nursing team, into an interdisciplinary plan of care.
• Is able to communicate this plan to all care providers in a way that ensures continuity of care for the patient/resident.
• Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, families, medical staff, and community
• Demonstrates the proficiency with computerized documentation.
• Demonstrates responsibility for one's own practice, utilizing educational opportunities to expand or refresh skills and/or knowledge and then demonstrates the ability to apply knowledge and share with others.
• Demonstrates the ability to provide education to patients/residents regarding their healthcare issues and care plan strategies. Provides ongoing education regarding nursing practices to other members of the healthcare team under his/her supervision
• Demonstrates the ability to document data in an accurate and timely way that maintains the facility resources to meet facility, state and federal requirements.
• Complies with legal requirements, standards, policies and procedures, including, but not limited to those within the Compliance Process, Code of Conduct and HIPPA.

• Current, unencumbered, license in the Commonwealth of Massachusetts
• Proficiency in basic computer skills
• Strong attention to detail
