As a Weekend Home Health Nurse, you'll provide skilled nursing services during scheduled home visits.

Responsibilities include:

Perform skilled nursing visits in patients' homes.

Provide wound care, dressing changes, and wound assessments within your scope of practice.

Administer prescribed medications and treatments.

Assist with diabetes management, including blood glucose monitoring, insulin administration, and patient education.

Monitor patient conditions and communicate significant changes to the appropriate clinical team members.

Assess patient needs and implement established plans of care within your professional scope.

Educate patients and caregivers on medications, chronic disease management, and preventive care.

Maintain accurate, timely clinical documentation.

Collaborate with physicians, clinical supervisors, and interdisciplinary team members.

Promote patient safety, dignity, comfort, and independence.

Qualifications:

Active Massachusetts RN or LPN license in good standing.

Strong clinical assessment and communication skills appropriate to your license.

Experience with medication administration, wound care, and chronic disease management.

Ability to work independently while maintaining effective communication with the clinical team.

Strong documentation and time-management skills.

Reliable transportation and willingness to travel to patients' homes.

Home health or community nursing experience preferred.
