Role Overview
An exciting new role has been created for a motivated and patient‑centered senior clinical pharmacist to utilise their clinical skills and work with multidisciplinary colleges across the Integrated Care System (ICS) to provide coordinated care to patients, reduce hospital re‑admissions, support safe discharge, transfer of care, and improve medicines safety and compliance in Worcestershire new Hospital at Home service. A confident communicator with strong organisational skills who thrives in an MDT environment, you will be instrumental in supporting and developing this new service to ensure a high‑quality service for our patients.
You will need to be a confident decision maker, with clear leadership skills, excellent pharmaceutical knowledge and technical skills, and a sound understanding of pharmacy service and hospital service. Main duties include:
Lead, deliver, develop, and assess the value of clinical pharmacy services to clinical pharmacy support to Hospital at Home service. Support the development of good practice and safe medicines management and optimisation systems across Herefordshire and Worcestershire Integrated Care System (ICS). Work in collaboration with different organisations across the ICs to provide highly effective clinical pharmacy services to patients within Hospital at Home service.
Develop and work within appropriate governance structures for the designated clinical service, liaising with IC‑wide stakeholders as needed.
