Community Mental Health Nurse - Older Adult

Herefordshire and Worcestershire Health and Care NHS Trust. The Older Adults Duty and Triage Team offers a full-time band 6 Registered Community Mental Health Nurse role that works predominantly with people aged over 65 who have dementia or complex mental health problems and covers all of Worcestershire.
The post requires collaboration with a cohesive team to manage and triage incoming referrals to the service. Referrals may be signposted to other agencies or assigned to a community mental health team for assessment and treatment. The role also involves visiting patients who require urgent input to complete an initial assessment and arrange appropriate follow‑up.
Experience with the patient group and community work, and a sound knowledge of risk assessment, is required. One‑to‑one supervision and ongoing support will be provided.
The role is 37.5 hours per week, Monday to Friday 9.00 am to 5.00 pm. Flexible arrangements can be made for part‑time work (minimum 22.5 hours per week).
Main duties of the job- Undertake urgent assessments of mental health in various community settings, including risk assessment.
- Triage paper referrals and conduct telephone triage to determine the most appropriate service for ongoing mental health support.
- Access and record information from various databases such as Care Notes, ICE, and Adult Social Care records.
- Conduct professional telephone conversations with partner agencies, patients, relatives, and carers.
- Arrange admissions to hospital when assessment requires it and liaise with the Approved Mental Health Professional Service for admissions under the Mental Health Act.
- Liaise with Community Area Consultant Psychiatrists as required.
- Liaise and refer to our Hospital at Home Team, including joint visits when indicated.
- The role does not involve holding patient cases for any prolonged intervention.
- Able to meet the travel requirements of the role.
- Ability to communicate clearly and effectively in English, both verbal and written.
- Ability to work autonomously and set own priorities.
- Supervisory/appraisal skills.
- Ability to organise and respond efficiently to complex information.
- Ability to use a variety of IT systems and databases.
- Specialist knowledge and application of interventions relevant to the patient group.
- Knowledge/experience of audit procedures.
- Detailed knowledge of mental health legislation and current practice.
- Detailed knowledge of the principles of clinical governance and its application.
- Knowledge of Person Centre Care.
- Knowledge of Risk Assessment.
- Multi‑disciplinary team working.
- Knowledge and application of standardised assessments.
- Registered Mental Nurse (RMN) (degree/diploma and working towards degree).
- Evidence of Continuing Professional Development.
- Original certificates to verify qualifications at the point of offer.
- Documented evidence of continuing professional development.
- Significant post‑registration experience including experience within mental health.
- Minimum 1 year within a related specialty.
- Extensive clinical experience including individual and group work.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a submission for Disclosure and Barring Service check to ensure no previous criminal convictions.

Salary: £39,959 to £48,117 a year, pro rata for part time.
