We have an exciting opportunity for a Band 5 Occupational Therapist to join SHAPE (Supporting Health and Promoting Exercise).

The primary purpose of this role is to support the delivery of a health and exercise programme for patients on the Severe Mental Illness Register and to support the wider team in ensuring the annual physical health checks through OT assessment and care planning for those harder to reach patients. This full‑time post works Monday to Friday and the post holder will be based at Studdert Kennedy House, supporting the delivery of SHAPE in several locations across Herefordshire and Worcestershire.

Responsibilities
  • Provision of occupational therapy assessments, addressing occupational performance and skill deficit and enabling the client to attend groups and physical health check appointments.
  • Work with clients to identify occupational therapy goals as part of the overall care plan, taking on the role of care co‑ordinator where relevant.
  • Plan and implement client‑centred individual and/or group interventions in collaboration with the client, using graded activity to achieve therapeutic goals.
  • Monitor, evaluate and modify treatment for clients with multiple needs in order to measure progress and ensure effectiveness of intervention.
  • Support patients in accessing their annual physical health check.
  • Engage in regular physical activity.
  • Joint working and planning with stakeholders and voluntary sector providers, signposting patients to other services, VCSE and specialist providers where appropriate.
Qualifications

Applicants should meet the necessary qualifications for a Band 5 Occupational Therapist.
