We are excited to offer an opportunity to join Worcestershire County Council's Urgent Care Team (Discharge Team). We currently have vacancies within our teams based at Worcester Royal Hospital, supporting adults with complex discharge planning in both hospital and community settings.

The Urgent Care Team operates a 7-day service, and there will be opportunities to work weekends and bank holidays on a rota basis. Enhanced payments are available for Sunday and bank holiday working.
About The TeamOur team works closely with colleagues across the Acute Trust, Health and Care Trust, community services, and other Worcestershire County Council teams to support timely, safe, and person-centred discharges. We are committed to the Home First approach, promoting independence and ensuring people can return to their own homes whenever possible.
Using a strengths-based approach and working within the framework of the Care Act 2014, we support individuals to identify their goals, build on their strengths, and achieve positive outcomes.
We are passionate about ensuring that people remain at the centre of decision‑making. The principles of the Mental Capacity Act 2005, least restrictive practice, and individual choice underpin all aspects of our work.
AboutThe Role
As a Social Care Worker, you will manage your own caseload with the support of an experienced and knowledgeable team. You will play a key role in supporting complex hospital discharges, facilitating safe transitions from hospital to community settings, and helping individuals achieve the best possible outcomes.
- Supporting adults through complex discharge planning and care coordination.
- Completing strengths‑based assessments and contributing to person‑centred support planning.
- Identifying and responding appropriately to safeguarding concerns, including adults who may be at risk of abuse or neglect.
- Working within the principles of the Mental Capacity Act, including supporting capacity assessments and best‑interest decision‑making.
- Collaborating effectively within a multidisciplinary team environment.
- Promoting independence, wellbeing, and the Home First principle in all aspects of practice.
- Maintaining accurate records and ensuring compliance with relevant legislation and professional standards.
