Conduct mental health assessments to evaluate patients' needs and diagnoses
Develop individualised care plans and treatment strategies for patients in the community
Provide counselling, psychotherapy, and support to individuals with mental health conditions
Administer and monitor psychotropic medications, educating patients about their use and side effects
Offer crisis intervention and de-escalation techniques during mental health emergencies
Collaborate with other healthcare professionals, social workers, and community resources to coordinate care
Advocate for patients' rights and access to appropriate mental health services
Educate patients, families, and the community about mental health issues, prevention, and available resources
Conduct home visits and community outreach to provide care in familiar environments
Monitor and assess patients' progress and adjust treatment plans as necessary to promote recovery and well-being
