The post holder is an experienced Advanced Nurse/Clinical Practitioner who will work as a member of the Practice Clinical Team. The post holder will have completed or is working towards the MSc Advanced Clinical Practice Masters Degree or Degree level qualification and relevant experience and RCN Credentials. The post holder will be supported by a named practice mentor.
Patient centred care will be delivered by triagingand assessing patients with undifferentiated and undiagnosed problems, initiate treatment, investigations, advice and management of problems using professional autonomous decision making processes, liaising when appropriate with other healthcare professionals and service providers to ensure safe and effective patient care.
Provides appropriate patient care in accordance with practice protocols and policies. Actively engages patients and carers in management of their health, promoting health and wellbeing.
They will work collaboratively with themulti-disciplinary general practice and primary care teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. To work to this level NMC requirement for advanced practice must be met.
Main duties of the jobThe post holder is expected to:
Adhere to NCPC policies and procedures and relevant legislation including the requirements of the any professional bodies
Attend mandatory training as identified by the organisation.
Adhere to NCPC infection prevention policies,procedures, audits in line with the Health Act 2006, to actively reduce Health Care Associated Infections

Jobresponsibilities Communication
- Refers when necessary to other external sources including Accident and Emergency, Secondary care, District Nurses, Emergency Social workers, Emergency mental health team, and other Emergency Services.
- Basic life support, severe breathing problems, serious cardiovascular problems, collapsed patients and seriously ill patients.
- Recognition of abnormal vital signs observing paediatric and adult respiratory rate and effort, heart rate rhythm and volume, temperature and blood pressure as appropriate.
- Perform investigative procedures including, Performing ECGs, Urinalysis, Blood Glucose monitoring, Venepuncture, measuring and interpreting peak low.
- Examination of the Ear Nose and Throat system
- Basic Neurological examination of Headaches and Head Injury
- Basic Musculo-skeletal examination in the assessment of Injury
- Basic eye examination in the assessment of injury, foreign body and infection.
- Respiratory Examination
- Abdominal Examination
- Cardiovascular Examination
- Vaginal Examination and sexual health screening
