Reablement Support Workers will be responsible for working with people identified with reablement needs to ensure key accountabilities.
Driving licence is essential.
Job Accountabilities- Reablement goals/outcomes are met in accordance with the reablement plan.
- Any additional goals/outcomes that may improve quality of life and promote independence are identified during the reablement episode.
- Feedback about additional identified goals/outcomes is fed back to the Reablement and Assessment Officer.
- Clear and timely communication with the Reablement and Assessment Officer in relation to the individuals identified goals/outcomes being met.
- Close team working within a small dedicated team of reablement support workers, communicating effectively about progress or concerns in achieving goals.

DBS ChecksThis role requires a DBS check. The level of check for this role is Enhanced DBS Check with an adults Barred List check.
