This role plays a key part in shaping and strengthening a therapy‑led reablement service aligned with national best practice. You will support Reablement Managers to enhance the therapeutic offer, designing, implementing and embedding meaningful improvements that deliver real outcomes for individuals.

• Support Reablement Managers to enhance the therapeutic offer, designing, implementing and embedding meaningful improvements.
• Lead change within a dynamic environment, driving service development and overseeing agreed improvement actions.
• Collect and evidence the impact of change to deliver real outcomes for individuals.
• Develop graded, therapy‑led programmes that promote independence, maximise individual strengths and improve quality of life, supporting prevention and long‑term wellbeing.
• Champion the service, building positive relationships with internal and external stakeholders.
• Inspire and guide managers and frontline staff to create lasting improvements.

• Proven leadership skills and experience in service improvement or change management.
• Commitment to person‑centred outcomes and continuous improvement within a reablement or related service.
• Strong communication and relationship‑building skills.

Enhanced DBS check with an Adults Barred List check is required.
