Position: PCN Additional Roles Clinician

We are seeking an experienced and motivated clinician to join our multidisciplinary primary care team. The post holder will independently assess, diagnose and manage patients with acute, chronic and complex medical conditions, delivering safe, high-quality, patient-centred care.

This role offers a varied clinical workload including same-day presentations, routine on-site appointments and home visits for housebound patients following GP triage. On designated visiting days, up to 10 home visits will be undertaken between 10:00am and 6:00pm, with 45 minutes allocated per visit. When working on site, the clinician will manage up to 26 patients per day with 15-minute appointment slots.

The post holder will work collaboratively with GPs, nursing colleagues and wider primary and care services to ensure coordinated and effective care. Where the clinician is not an independent prescriber, they will provide medication recommendations and work closely with a prescribing clinician.

Appropriate training, supervision and mentoring will be provided.

Main duties of the job

The post holder will provide first-contact care for patients presenting with undifferentiated acute and chronic conditions, undertaking clinical assessment, diagnosis, and management planning. They will develop and deliver patient-centred care plans, interpret diagnostic tests, and implement appropriate treatment, including health promotion and self-management support.

The role includes conducting home visits, and working collaboratively within the multidisciplinary team and external services to ensure coordinated care. The post holder will maintain accurate clinical records, participate in clinical governance, audit, and service improvement activities, and support enhanced services and organisational priorities.

They may also contribute to teaching, mentoring, and quality initiatives such as QOF and other incentive schemes, while engaging in ongoing professional development.

Job responsibilities

Primary responsibilities:

Provide first point of contact care for patients presenting with undifferentiated, acute and chronic conditions

Undertake history taking, physical examinations and clinical decision-making to establish working diagnoses and management plans

Develop and implement patient-centred care plans in partnership with patients and carers

Analyse and act upon diagnostic test results

Undertake home visits as part of a rota system

Receive referrals and direct patients from triage services and other clinicians

Plan, implement and evaluate treatment and interventions for patients with acute and chronic conditions

Utilise non-pharmacological therapies and, where appropriate, consider pharmacological interventions in line with national and local guidance

Prioritise and manage urgent or emergency situations, initiating escalation where required

Support patients to adopt health promotion strategies and self-care

Work collaboratively with the multidisciplinary team and external services

Maintain accurate, contemporaneous and complete clinical records

Participate in clinical audit, significant event reviews and clinical governance activity

Support enhanced services and other service requirements on behalf of the organisation

Participate in appraisal, supervision and personal development planning

Support delivery of QOF, incentive schemes and service improvement initiatives
