Senior Registered Nurse

Responsibilities

• To work as part of Southern Hospice Group multi-disciplinary team providing high quality Palliative and End of Life nursing care for patients and their families in accordance with the NMC guidance and Southern Hospice Group policies and procedures.
• Partner with District Nursing teams and Continuing Healthcare funded agencies to support the practical care of patients at end of life in the community.
• Assess and monitor changes in a patients condition, advising on complex symptom control measures and implementing changes according to best practice and community and/or hospice policies.
• Provide effective and sensitive communication between all community and hospice professionals, as well as patients, carers and relatives.
• Administer medicines within assessed competency and hospice guidelines according to the needs of the patient.
• Undertake risk assessments in the community to ensure safe practice, including identifying appropriate skill mix and lone worker issues.
