The Department of Developmental Services (DDS) is seeking an enthusiastic, compassionate, and reliable nursing professional to fill a Licensed Practical Nurse II position on the Evaluation and Stabilization Program (ESP) in Wrentham, MA. The role involves providing services and supports to individuals with intellectual and developmental disabilities who may exhibit challenging and extreme behaviors, ensuring an environment that meets each individual's needs in daily living, active treatment, and habilitative and behavioral programs.
Specialized training is required.
- Provide exceptional nursing care and oversight, including evaluation of behavioral and episodic health needs through assessment, continuous observation, and monitoring of intervention changes.
- Administer medications and treatments according to provider orders and established policies.
- Safeguard controlled substances to prevent tampering or diversion.
- Monitor equipment functionality and ensure supplies and medications are available.
- Perform all clinical tasks to accepted nursing standards, ensuring safe and effective care delivery.
- Advocate for individuals, showing respect and positive regard toward those with developmental disabilities.
- Support individuals on the behavioral unit exhibiting challenging and extreme behaviors.
- Monitor health and safety, adhere to rules and regulations, and communicate concerns to nursing and DSW supervisors.
- Seek supervisory assistance when needed, including for administrative issues and evolving clinical presentations.
- Assist in planning, coordinating, and implementing care for acutely ill or injured individuals; act as a first responder during emergencies or crises.
- Complete daily reports and document incidents that occur during the shift.
- PABC Certified and/or trainer
- Experience with challenging behaviors and PBS implementation across settings
- Ability to remain calm in high‑stress environments
- Excellent time‑management and communication skills
- Professional and respectful relationships with staff and individuals
- Effective interaction with other staff and supervisors
- Debriefing after stressful situations
- Skilled in Microsoft operating systems
- Leadership and direction for direct care staff
- Physical ability to assist/lift individuals with disabilities or limited mobility
- English reading and comprehension skills
- Team‑building and supportive attitude
- Knowledge of standard nursing policies, procedures, and techniques
LPNs applying for this assignment must successfully complete the Proactive Approaches to Behavioral Challenges (PABC) training within 6 months and maintain PABC certification. All candidates must also hold current CPR certification.
Position Requirements- Current, active LPN license in Massachusetts
- Current, valid Class D driver’s license
- Health Care Provider BASIC LIFE SUPPORT (BLS) certification
Some positions may require 24/7 staffing; mandatory overtime may be requested. This role is designated as an “EMERGENCY” position, requiring report when a state emergency is declared.
