Support Worker, Autism Hub Team - Channel Islands - Sanctuary Personal

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Support Worker within the Autism Hub Team to work Full Time based in Guernsey.

The salary for this permanent Support Worker job is up to £34,356per annum.

• To support patients/service users in undertaking activities of daily living within care plans.
• To monitor and report changes in patient/service users presentation to the staff member in charge and take appropriate action.
• To report issues of concern in line with policy and contribute to any Quality & Safety processes.
• To build a rapport with patients/service users and their support network, communicating with them in a way that respects their views.
• To use a wide range of communication skills that can be adapted to meet the needs of complex challenging situations.
• To demonstrate effective communication skills with all members of the Multi-Disciplinary Team.
• To provide clear, concise, and accurate electronic and paper records of the care given in accordance with agreed policy and standards.
• Communicates to patients/service users and relatives compassionately, in a confidential and sensitive professional manner.
• To promote high quality care through the role modelling of professional behaviours.
• To contribute to effective team working through communicating with others in a constructive way offering support and assistance and making suggestions for the improvement of care delivery.
• To attend and participate in team meetings.
• To understand the role of a range of professionals from other service areas who may be participating in the care of a patient/service user.
• To complete training in accordance with the statutory, mandatory and essential to role training policy.
• To demonstrate the ability to develop new knowledge and skills, applying this to practice, and share with colleagues as appropriate.

• Approved programme of development at Diploma Level (Level 5 HE) or equivalent e.g. Level 4 BTEC Professional Certificate in Positive Behavioural Support
• Understanding of relevant legislation.
• Registered or eligible for registration.
