Experienced Social Worker - Team for Older People
37 hours per week.
We are looking to recruit an experienced, committed, creative and resilient Social Worker to support our continuous improvement journey towards best practice, evidenced by our recent positive inspection.
About the RoleAs an Adult Social Worker in the Team for Older People, you will support adults in the community, care homes, and hospital settings, working within a broad and diverse remit to deliver person‑centred care.
Responsibilities- Multi-disciplinary working with families, unpaid carers, third sector and health professionals.
- Assistive technology and support for unpaid carers.
- Reablement and direct payments.
- Working with people living with Dementia diagnoses.
- Hospital discharge and hospital avoidance.
- Safeguarding and self‑neglect.
- Mental Capacity and Best Interest decision‑making.
- Continuing Healthcare assessments.
- Intervention under the Mental Health Act.
- Court of Protection.
- Allocation of a supervisor and mentor, regular peer support, robust supervision and management support.
- Attendance at regular team and locality meetings.
Relevant professional qualification in social work, commitment to supporting adults to achieve personal outcomes, and ability to work creatively and resiliently in a dynamic team.
