Job Overview

The post holder will work as an Advanced Practitioner Occupational Therapist within Primary Care, providing specialist assessment, formulation and intervention for individuals with complex mental health and psychosocial needs.

Working autonomously as a first‑contact practitioner, they will complete clinical assessments, treatment planning and evaluations to support timely access to appropriate care. Interventions will focus on improving occupational performance, functional ability, health and wellbeing using recovery‑focused, evidence‑based practice.

The post holder will manage a complex caseload and provide individual and group interventions across clinic, community and home settings. They will contribute to service development, audit, evaluation, clinical leadership, supervision, training and expert advice to multidisciplinary and multi‑agency teams.

The role supports integrated, person‑centred care through close partnership working with primary care, wider health services, local authorities and third‑sector organisations, contributing to proactive and preventative community‑based models of care.

Multiple WTE posts are available, with working patterns agreed in line with service and individual needs. The ability to speak Welsh is desirable;
English and/or Welsh speakers are equally welcome to apply.

Main duties of the job
  • Provide autonomous specialist occupational therapy assessment, formulation and intervention for patients presenting in primary care
  • Undertake first‑contact assessments, including history taking, clinical reasoning, care planning and evaluation
  • Manage and prioritise a complex caseload, delivering evidence‑based, person‑centred interventions
  • Promote function, independence and participation across daily living, work and social roles
  • Complete risk assessment and management, including positive risk‑taking
  • Deliver and evaluate individual and group interventions, adapting to patient need
  • Work across clinic, community and home settings
  • Communicate complex information effectively and support patient involvement in goal setting and self‑management
  • Work collaboratively with multidisciplinary and multi‑agency teams, including third sector partners
  • Provide clinical leadership, supervision and training to staff and students
  • Contribute to service development, audit, quality improvement and innovation
  • Maintain accurate clinical records and reports in line with professional and organisational standards
  • Adhere to HCPC/RCOT standards, clinical governance and CPD requirements
Person specification Qualifications and Experience Essential criteria
  • Recognised qualification degree / equivalent in Occupational Therapy
  • Accredited post‑graduate study relevant to clinical area at master’s level or equivalent
  • Broad and extensive experience in Occupational Therapy practice
  • Significant experience of Occupational Therapy delivery in specialist area
  • Experience of clinical leadership
Desirable criteria
  • Other professional, technical or academic qualification which relate to clinical practice
  • Motivational interviewing/ Solution Focused techniques
  • Student accreditation (Practice Placement Educator Certificate)
  • Clinical supervisory skills training
  • Experience in leading service development / improvement
Skills Essential criteria
  • Proven and well developed communication skills at a variety of levels
  • Demonstrable skills in influencing change in clinical practice
Desirable criteria
  • Welsh speaking
  • Skills in research
