Job Description:

We are seeking a compassionate, forward-thinking Registered Learning Disability Nurse (RNLD) to join our interdisciplinary team with a focus on our community care. In this dynamic role, you will bridge the gap between physical health, mental well-being, and behavioural support. You won't just be managing care; you'll be empowering clients and their families. Whether leading medical desensitisation programmes, supporting health action planning, or collaborating alongside Speech and Language Therapists, Occupational Therapists, and Behaviour Analysts, your expertise will directly transform lives.

Key Responsibilities:

  • Holistic Health Management:
    Conduct comprehensive health assessments and implement person-centred plans for individuals with complex learning disabilities and neurodivergent profiles.
  • Medical Desensitisation:
    Design and deliver structured desensitisation plans to support clients in accessing essential healthcare, medical appointments, and procedures.
  • Multidisciplinary

    Collaboration:

    Work closely with OT's, Behaviour Consultants and Behaviour Support Workers, to deliver integrated support packages.
  • PBS Integration:
    Apply Positive Behaviour Support principles, treating behavior as a key form of communication and advocating for proactive strategies.
  • Family & Team Guidance:
    Provide coaching, training, and emotional support to families, school staff, and support workers.
  • Clinical Governance:
    Maintain exemplary clinical documentation and ensure compliance with Nursing and Midwifery Council (NMC) and Care Inspectorate Wales (CIW) standards.

What we ask of you:
  • Qualifications:

    Active registration as a Registered Learning Disability Nurse (RNLD) with the NMC.
  • Experience:

    Experience working with individuals with learning disabilities, autism, and behaviours that challenge.
  • Clinical

    Skills:

    Knowledge of physical health management, medication administration, health desensitisation, and health action planning.
  • Mindset:
    An open-minded, collaborative professional who values evidence-based practice (PBS/ABA) and thrives in a multidisciplinary environment.
  • Flexibility:
    Full UK driving licence and access to a vehicle for community visits across the region.
Schedule:

Full-time (40 hours per week)

From £32,000 to £36,000 per year

Ability to commute/relocate:

Pembrokeshire: reliably commute or plan to relocate before starting work (required)
