- Provide support and guidance to all participants that have been registered onto the project.
- To develop ongoing action plans with participants which will be reviewed on a regular basis.
- To provide one to one support and group activities to aid the development of a participant in their recovery.
- Work alongside the team structure in delivering services and outcomes for the scheme.
- To achieve project targets and outcomes set by Welsh Government.
- To support the team leader and referral agents in raising awareness of the service.
- To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance misuse.
- Provide support and supervision to volunteers.
- Darparu cymorth un-i-un a gweithgareddau grŵp i gynorthwyo datblygiad cyfranogwr yn ei cynllun.
- Cyrraedd targedau a chanlyniadau'r prosiect a osodwyd gan Lywodraeth Cymru.
