Job Overview
Betsi Cadwaladr University Health Board (BCUHB) North Wales - Patient Flow/Discharge Coordinator position for East Mental Health & Learning Disabilities (MHLD) Team based at Heddfan Unit in Wrexham. This role supports safe and effective operational management of inpatient capacity and discharge planning for mental health patients within the East locality.
Main duties of the jobThe successful candidate will lead and manage patient flow to ensure a seamless service. The post requires substantial experience as a complex discharge nurse and the ability to coordinate and supervise a multidisciplinary team.
Qualifications & Knowledge- Registered Mental Health nurse 1st Level (RMN).
- Evidence of ongoing professional development and training.
- Degree or Diploma as a registered nurse 1st Level.
- NMC Registration.
- Evidence of nursing related qualification to diploma or degree level.
- Postgraduate diploma or equivalent level experience.
- Experience with in the adult/older person mental health field.
- Teaching/assessing qualification to Postgraduate Diploma Level.
- Welsh speaking ability – Welsh and/or English speakers are equally welcome.
- Presentation skills.
Essential criteria
- Experience in discharge planning, health care eligibility criteria, and legal aspects of discharge pathways.
- Working knowledge of multidisciplinary team collaboration.
- Clinical credibility.
- Experience working autonomously and as part of a team.
Desirable criteria
- Evidence of Discharge Process and MDT/CRT working.
Essential criteria
- Excellent communication & interpersonal skills.
- Ability to form positive working relationships across sectors.
- Ability to prioritise own workload.
- Self-motivated and a positive role model for all disciplines.
- Able to present relevant information concisely.
- Acts in line with legislation, policies, and procedures, and adheres to HB Health & Safety policies.
- Demonstrates enthusiasm towards teaching and sharing knowledge.
- IT literate and able to extract key points from numerical data.
- Ability to set and meet own targets.
- Organisational and time‑management skills.
- Accuracy in patient record keeping.
- Effective leadership and management experience.
- Diplomacy and tact in working with others.
- Experience delivering presentations to groups.
- Self‑motivated, able to motivate and support others.
- Adaptability to change.
Desirable criteria
- Presentation skills.
- Welsh speaker.
- Welsh language proficiency levels 1 to 5.
Essential criteria
- Satisfactory Enhanced DBS clearance with Adults.
