Key Responsibilities

  • Patient Admissions and Discharge Take the lead in ensuring safe and timely admission and discharge of patients, actively identifying and addressing any delays in the discharge process.
  • Communication and Advocacy:
    Act as an advocate for patients, carers, and relatives to enhance their admission and discharge experience. Facilitate sensitive discussions regarding admission and discharge plans, addressing any disagreements and ensuring all parties are informed and aligned Multidisciplinary Collaboration Chair multidisciplinary admission and discharge meetings, ensuring that all relevant stakeholders are engaged in the discharge planning process.
  • Patient-Centred admission and discharge planning Collaborate with the clinical staff to develop and implement realistic and up-to-date admission and discharge plans that are centred around the individual needs of each patient. Monitor and amend plans as necessary to minimise potential delays.
  • Triage and Admissions Support Record Keeping Maintain concise and up-to-date records of admission and discharge plans and patient interactions using relevant, paper and electronic systems.
  • Education and Support Provide guidance and education to ward staff regarding current legislation, admission and discharge planning best practices, and adherence to NICE guidelines.
  • Data Collection and Audits Participate in the collection and presentation of data related to patient admission and discharge and support the development and maintenance of discharge documentation.
Qualifications and Experience
  • Registered Nurse.
  • Significant clinical experience.
  • Strong understanding of discharge planning processes and familiarity with relevant legislation and guidelines.
  • Excellent communication and interpersonal skills, with the ability to navigate sensitive conversations and advocate effectively for patients.
  • Demonstrated ability to work autonomously and manage competing priorities in a fast-paced environment.
  • Proficient in using electronic health record systems for documentation and information sharing.

It is a requirement that you adhere to Nightingale House Hospice Policies, Procedures, Protocols and guidelines always.
